If you are out of network or do not have insurance, we offer a private pay option for individual nutrition counseling services. Your specific plan may or may not cover nutrition counseling.
It is your responsibility to know and understand the specific benefits of your insurance plan.
Insurance Script
Prior to your first appointment with Monadnock Nutrition Services, we require that you verify the coverage of nutritional counseling benefits with your insurance company. As a member, it is your responsibility to know and understand the benefits of your insurance plan. If your insurance denies coverage after any session(s) with Monadnock Nutrition Services, you are financially responsible for the denied claim. Below you will find an insurance script as a helpful guide to inquire about your specific benefits. Please note that all questions must be answered and returned to us prior to your initial appointment.
